Absence Report

This form must be submitted to the TAMS Academic Office within 36 hours of your absence.  If the absence occurs on a Friday you must submit it by 12:00 noon the following Monday.  If not, the absence will be un excused and you will receive 15 disciplinary points.  Faculty will also be reporting absences.  If you have failed to report an absence that is reported by your instructor you will be given 15 disciplinary points.
