Each student is allowed to sign up for up to two individual tests (a math test and a science test). Before completing this section, it might be helpful to first make a list of students and their test preferences. Also, indicate how many teams (each up to 6 members) you will bring.

The registration fee is $10 per student and includes lunch and a T-shirt. It is payable by cash or check (to "TAMS Mu Alpha Theta") and can be paid during the event. Sponsors receive a complimentary lunch and a T-shirt.

Please list the names of all students participating, their grade level, which individual exams they plan to take, and their T-shirt size, separated by commas. We will be using this as reference for scoring sweepstakes points if a student forgets to put down his or her school name on the answer sheet. Use the abbreviations above for the exams. Also, indicate the sponsor's T-shirt size.

If you've accidentally made a mistake, don't hesitate to submit again. Beside the school name, put "—NEW" or something similar. If your team information changes, please update us by sending an email to tamstournament@gmail.com.
